1). Print and complete the Centennial SD ACT 372 form
2). Scan completed ACT 372 Form and email to [email protected]
3). On the email subject line, please include the following- Attn: Centennial Bus Request, the student(s) names and NEW student(s) bus rider(s).
Returning Student Bus Riders;
1). Print and complete the Centennial SD ACT 372 Form
2). Scan completed ACT 372 Form and email [email protected] and please copy [email protected] on the same email.
3). On the email subject line, please include the following- Attn: Centennial Bus Request, the sudent(s) name(s) and RETURNING student(s) Bus rider(s).
